Agencies wishing to make a referral should follow this procedure:

1. Contact the Little Sand Intake Worker 218.566.2342
2. Send written referral material including Social History, most recent Psychological Evaluation, previous placement history, and other pertinent information.
3. Should Little Sand accept a referral for admission, an intake packet

4. Upon admission, a meeting will be held for intake packet review and treatment planning.

Fees - Daily Per Diem negotiated through Host County Contract.

LSGH is a Title IV-E Facility.
